TELEHEALTH

A phone telehealth consult involves a phone call to your personal number from a GP if you have been to our surgery in the past 12 months.

Phone consultations are not available to new patients.

Private fee´s do apply. Medicare offers rebates for phone telehealth appointment.

If you have a Pension or Centrelink concession card this service will be bulk billed.

Where fee´s do apply payment must be made over the phone after your consultations.

We are providing COVID-19 vaccinations to all patients 12 years and older.

Booster vaccine are now available for anyone over 18 years old.

If you are a new patient, you will need a copy of your health summary from your regular GP, photo ID and your Medicare card.

MENTAL HEALTH PLANS

A Mental Health Care Plan (MHCP) is a referral provided by a GP to support patients to access mental health support. With a MHCP, patients can access up to 20 Medicare-subsidised psychology sessions per calendar year.

WHO IS ELIGIBLE?

Your GP can determine if you are eligible for a Mental Health Care Plan.

Eligible patients can receive an MHCP once every 12 months, which entitles them to up to 20 Medicare-subsidised psychology sessions.

After the first 6 sessions, the patient must book an appointment with their GP for review, before any additional sessions can be accessed. Due to COVID-19, Medicare has enabled patients to access an additional 10 sessions. GP review is required after the 10th psychology appointment.

HOW DO I GET ONE?

To obtain an MHCP you need to make a ‘Mental Health – Initial Consultation‘ appointment with your usual GP.

To ensure continuity of care, it is important that you continue to visit your usual GP for your initial MHCP and any reviews of your MHCP.

CARE PLANS

An Enhanced Primary Care Plan (EPC) is a referral that is written by your GP which provides eligible patients with Medicare-subsidised allied health appointments, like physio and podiatry. An EPC may also be referred to as a GP Management Plan (GPMP).

WHO IS ELIGIBLE?

To be eligible for an EPC or GPMP, a patient must have a chronic or terminal medical condition with or without complex care needs.

A chronic health condition is defined as one that has been present for 6 months or more. It includes, but is not limited to, asthma, cancer, cardiovascular illness, diabetes mellitus, musculoskeletal conditions and stroke.

Your GP will need to assess your eligibility for an EPC plan.

HOW DO I GET ONE?

To obtain an EPC or GPMP you need to make a standard appointment with your GP.

Please note that this referral takes 15 minutes, and cannot be done in a standard appointment when other issues are being discussed. If you are coming to see your GP about another issue, you will be required to book a long (30 minute) appointment or come back for another standard appointment.

To ensure continuity of care, it is important that you continue to visit your usual GP for your initial EPC plan and any reviews of your EPC plan.
