If you are a new patient to our clinic, please download and fill this “New Patient Form” before you come to clinic to save your time.
Your first appointment with us will be longer than usual. Either the practice nurse or doctor will need to gather a full history to help ensure we provide you with appropriate health care. Your history includes gathering pertinent information such as medicine and food allergies, current medications, your family history, and your past medical history.
Medicare Card and Health Insurance Information
On your first visit please bring the following (if applicable): Medicare card, DVA card and concession card.
Visits to our doctors are privately billed. These costs will be discussed with you in advance. Payment is required at the time of consultation. Please have a look in About us for our fee structure.
Records Release Form
If you would like your medical records transferred to the SCMC, ask us for a records release form. This form is used to give another doctor permission to transfer your medical records to our clinic. It is important that we have as complete a record as possible of your health history, vaccine records, and past pathology results and letters from specialists.

Medicare Rebate transferred to your account
We accept cash, EFTPOS, Cheque from regular clients and all major credit cards (VISA & Mastercard) with no handling fee. We don’t accept AMEX / AMERICAN EXPRESS.
Health Funds & Rebates
Our consultation rebates are in accordance with Medicare Australia’s rebate for eligible services. Rebates are also available from many private Health Funds and can be claimed by using the tax invoice issued by SCMC. Check with your Health Fund to see whether you have coverage for Allied Health Services.
